How to update property-owner information
To update property-owner’s address you must complete the following documentation:
To update agent information you must complete the following documentation:
- Official signed letter from landlord indicating requested changes or Agent Contract
- W-9 form (only if agent is listed as the payee)
- Agent Affidavit (If applicable)
- Assignment of New HAP Contract (only if agent is listed as the payee)
To update banking information:
Owner of Property Transition
If there is a change in the owner of the property or the Tax Identification Number, the following requirements must be completed.
Note: The new owner should reference How to become a participating Housing Choice Voucher (HCV) landlord to ensure he/she is in compliance with all PHA requirements.
